Pads in normal underwear are a bad solution.
They shift, they bunch, they announce themselves through fabric, and they end up in the bin twice a day. Most people just plan around the problem instead, mapping every outing by where the bathrooms are. There is another way.
The build
The layer is inside the underwear.
An absorbent panel is sewn into the front, with a thin barrier layer behind it. Nothing to insert, nothing to reposition, nothing that moves during the day. From the outside it is a plain boxer brief, and it wears like one under jeans or a suit.

Questions
How much do they hold?
They are made for light leaks, the kind that come with a cough, a sneeze, or a delay getting to a bathroom. They are not built for heavy or continuous loss.
Can anyone tell?
No. The panel is thin and sits inside the front of the brief, so there is no outline under trousers and no sound when you move.
How many should I have?
Most people want at least three or four so there is always a clean pair while others are in the wash.
How long do they last?
Months of regular use if you wash cold and skip the softener and the dryer. Softener is the main thing that ruins them.
Should I see a doctor?
If this is new, getting worse, or came with pain or blood, yes. Leaks can have simple causes and treatable ones, and managing it quietly at home means nobody ever looks at why.
